The St. Johns Public Schools enrollment office is located in the Superintendent's Office.  This office is open for student enrollment from 7:30AM-4:00PM, Monday - Friday (hours may vary slightly in the summer). 

The parent/guardian must provide the following information for the student at the time of enrollment:

• An original certified birth certificate with the raised seal (NOT the original footprints from the hospital)
• Most recent immunization records (Note: if you need a copy, you may click on the link below to request a copy)
• Proof of residency verification

To expedite the enrollment process, you may print and complete the required forms below.  These forms are also available at the enrollment office.
